Sarcopenia is common in patients with end stage liver disease and liver transplantation. It negatively affects on the prognosis of both diseases. Therefore, sarcopenia in both diseases should be actively managed with rehabilitation team during whole process of liver transplantation. A 57-year old man had severe sarcopenia and gait disturbance after liver transplantation. We started rehabilitation with early mobilization strategy during his intensive care unit stay. After general ward transfer, gait training, ergometer training and resistive exercise were applied to him. His gait function was definitely improved after rehabilitation, and he could walk independently without assistive device after rehabilitation. However, muscle mass was not increased over two months rehabilitation.
